GLP-1 Medications, Muscle Loss & Joint Pain: How to Protect Your Musculoskeletal Health

GLP-1 medications such as semaglutide and tirzepatide have become powerful tools for weight management and metabolic health. But as with any significant weight-loss approach, there is an important consideration that often gets overlooked: preserving muscle while losing fat.

When muscle mass decreases too quickly, some people may notice changes in strength, stability, mobility, and how their joints feel. The good news is that there are proactive ways to support your muscles and joints while using a GLP-1 medication.


How Can GLP-1 Medications Contribute to Muscle Loss?

GLP-1 medications work in part by reducing appetite and food intake. This can make it easier to maintain a calorie deficit and lose weight. However, when overall food intake drops significantly, protein intake can also decrease.


Research shows that GLP-1 and related incretin medications generally result in substantially more fat loss than lean-mass loss, but some lean tissue is lost during treatment. A 2026 meta-analysis found that approximately 25–39% of weight lost in trials of incretin medications was lean mass, although lean mass does not necessarily equal skeletal muscle.


The risk can be greater when someone is eating very little protein or isn't performing resistance exercise.

Why Could Muscle Loss Lead to Joint Pain?

Your muscles play an important role in supporting and controlling your joints.


Think of your muscles as the body's built-in support system. Strong muscles around the hips, knees, shoulders, spine, and other joints help absorb forces and maintain proper movement.

When muscle strength or muscle mass decreases, several things can happen:

  • Reduced stability and control during movement

  • Increased stress placed on passive tissues such as tendons and ligaments

  • Changes in posture and movement mechanics

  • Greater difficulty performing activities that previously felt easy

  • Increased soreness or discomfort after exercise


This doesn't mean that GLP-1 medications directly cause joint damage. In fact, emerging research suggests GLP-1 medications may have potentially beneficial effects on inflammation and musculoskeletal health. The relationship between GLP-1 medications, muscle, and joint health is complex and still being studied.


Instead, rapid weight loss combined with inadequate nutrition and reduced muscle loading may contribute to musculoskeletal symptoms in some individuals.


Protein: One of the Most Important Pieces

One of the simplest ways to help protect muscle during weight loss is to make protein a priority.

Because GLP-1 medications can decrease appetite and cause early fullness, some people struggle to eat enough protein throughout the day. Current reviews commonly recommend individualized protein targets around 1.2 g/kg/day or higher for many adults undergoing significant weight loss, with higher targets sometimes appropriate depending on the individual.

Protein-rich foods include:

  • Chicken, turkey, and lean beef

  • Fish and seafood

  • Eggs

  • Greek yogurt

  • Cottage cheese

  • Milk

  • Beans and lentils

  • Protein shakes or other protein supplements

If getting enough protein through food is difficult because of decreased appetite, a protein supplement may be a convenient way to help meet nutritional needs.


What About Supplements?

Supplements should not replace a balanced diet, but certain supplements may be useful when appropriate.


Protein Powder

Protein powder can be particularly helpful for people who struggle to consume enough protein because of reduced appetite.


Creatine

Creatine is one of the most well-researched supplements for supporting strength and muscle performance. It may be worth discussing with your healthcare provider if you're using a GLP-1 medication and are concerned about preserving muscle.


Vitamin D

Vitamin D is important for bone and muscle health. If you're at risk for deficiency, your healthcare provider can determine whether testing or supplementation is appropriate.


Omega-3 Fatty Acids

Omega-3s may support overall cardiovascular and inflammatory health, although they should not be viewed as a substitute for strength training or adequate protein.

Emerging reviews have identified protein, creatine, leucine or essential amino acids, vitamin D, and other nutrients as potential adjuncts during GLP-1 therapy. However, the evidence is stronger for adequate protein and resistance exercise than it is for most individual supplements.


Always discuss supplements with your physician or qualified healthcare professional, particularly if you have kidney disease, take medications, are pregnant or breastfeeding, or have another medical condition.


Don't Forget Strength Training

Perhaps the most important strategy for maintaining muscle during weight loss is resistance training.


Your muscles need a reason to stay.


Strength training provides that stimulus by telling your body that muscle tissue is still needed. Research consistently supports resistance exercise as an important strategy for preserving lean mass during weight loss, including weight loss associated with incretin medications.

You don't necessarily need to spend hours in the gym. A well-designed program that includes exercises such as:

  • Squats

  • Lunges

  • Rows

  • Presses

  • Hip hinges

  • Deadlifts

  • Core exercises

can help maintain strength and functional movement.


How Can Chiropractic Care Help?

If you're taking a GLP-1 medication and you've noticed new joint pain, weakness, muscle soreness, or difficulty with activities that used to feel easy, don't automatically assume it's something you simply have to live with.


A comprehensive approach that includes adequate protein, resistance training, appropriate supplementation, mobility work, and individualized chiropractic care may help you maintain a stronger, more resilient musculoskeletal system throughout your weight-loss journey.
